Have You Turned in Your Forms for the 2019-2020 School Year? Plus, Info about Middle School Camp-Out

There are several forms that student families need to complete for the current year, even if they have been completed in previous years. We will collect forms at Back-to-School Night on August 22, or they can be turned in at the Grove School office. Please make sure that you turn in yours!

All families:

• 2019-2020 Updated Emergency Contact Form
• 2019-2020 Annual Permission Slip

Middle School Families:

• Middle School Camp-out Permission Slip – See slip and watch for communications with more details
• For Middle School students requiring medication during the camp-out, please complete an Authorization for Administration of Medication (must be re-submitted annually)

Families with students that will participate on athletic teams:

• Athletic Packet (must be re-submitted annually)

Other important forms:

• Free and Reduced Lunch Application (must be re-submitted annually)
• Authorization for Administration of Medication (must be re-submitted annually)
• Authorization for Self-Administration of Medication (must be re-submitted annually)
